All NHS New Member Applications are due by Monday, February 23rd, 2026 at 10AM.

To apply for NHS Membership, you must complete the 2026-2027 NHS Membership Application Form, which can be found HERE.

All applicants must fill out the form in its entirety, including two forms of leadership: 

• One MSD Staff Leadership Form - Click HERE to Download this Form: This could be a club officer position, a team captain role, or any other leadership within a school-sponsored activity. This may also be a teacher, but you must have shown leadership within the classroom/club. 

• One Community Leadership Form - Click HERE to Download this Form: This could be involvement in a volunteer organization, a leadership position in a community group, or any other significant leadership role outside of school. ​
